How To Take The Perkins Experience Survey
Step 1. Visit perkinsexperiencesurvey.com.
Step 2. You will need to have your Perkins Bakery & Restaurant receipt with you in order to proceed. You will need the 15 digit survey code, which can be found near the bottom of the receipt just above the words “Validation Code”. If somehow you’re unable to find the 15 digit survey code, you may click on the link “click here”.
Step 3. You will need to enter the following information in the next page- Store Number, Check Number, the Date and Time as listed on the receipt. These details can be found near the top of the receipt, with Chk # and the date and time just below it.
Step 4. You will be taken to the Perkins guest satisfaction survey. Make sure to answer all the necessary questions as they come up. Don’t leave any fields blank in order to be successful. Like other surveys, Perkins Experience Survey will ask you about the quality of the food, the place and the service you received during your visit. Answer all questions as honestly as possible so you can help improve the restaurant’s service.
Step 5. You will be required to enter personal details at the end. Once the survey is complete, you will be given a validation code at the end. It’s recommended to write down the code in the receipt and present it to any authorized Perkins employee to enjoy your prize.
